Quality Governance Framework

Build a lightweight Quality Governance framework covering standards, decision rights, evidence, quality gates, exceptions, escalation, release governance, and continuous improvement.

Governance should enable good decisions

Quality governance defines how an organization sets expectations, reviews evidence, manages exceptions, and makes quality-related decisions consistently.

Good governance reduces ambiguity. Bad governance adds approval layers without improving outcomes.

Core governance components

  • Quality policy/principles
  • Standards
  • Decision rights
  • Required evidence
  • Quality gates
  • Exception process
  • Escalation path
  • Metrics
  • Review cadence
  • Improvement backlog

Governance across the lifecycle

Intake / Planning ↓ Readiness / Testability ↓ Test Strategy Review ↓ Execution Evidence ↓ Release Readiness ↓ Production / Incident Review ↓ Improvement

Use risk-based governance

Not every change needs the same oversight. High-risk programs may require formal strategy review and sign-off, while low-risk product changes can rely on automated controls and team-level accountability.

Govern exceptions explicitly

Exceptions should identify the standard being bypassed, reason, compensating control, owner, expiry, and approval.

Avoid governance theater

A signed template with no evidence is not governance. Governance should improve decisions, traceability, and accountability.

Lead quality as an organizational capability

Connect strategy, operating models, governance, metrics, maturity, people, vendors, and delivery decisions.
