An escaped defect is evidence

A production defect should trigger more than one additional regression test. It provides evidence that part of the quality system failed to prevent, detect, or expose the risk.

Look for the missed control

Escaped Defect ↓ Requirement Gap? Coverage Gap? Environment Gap? Data Gap? Automation Gap? Monitoring Gap? Decision / Governance Gap?

Perform a quality-focused root-cause review

  • Was the expected behavior defined?
  • Was the risk known?
  • Was the scenario tested?
  • Was the environment representative?
  • Did test data include the condition?
  • Did automation miss it?
  • Could monitoring have detected it?
  • Was known risk accepted?
